A confident answer is dangerous when nobody can tell which document it came from.

The short answer
A trustworthy knowledge base agent retrieves from approved current sources, respects the requesting user's permissions, cites the material behind each answer and admits when evidence is missing or conflicting. Build an evaluation set from real questions, test retrieval and response separately, provide a human escalation path and assign owners to document freshness after launch.
The model can write a clear answer even when the source material is old, contradictory or outside the reader's access. That makes fluency a poor quality measure. A useful knowledge agent is an information system first: it knows which documents count, who may see them and how to show the evidence behind a response.
Start with the questions and the authority behind their answers.
Collect real questions from support, onboarding, operations and subject-matter experts. Group them by who asks, what decision follows and which source should settle the answer. A broad request to answer anything about the company produces an untestable system and hides where the underlying documentation is incomplete.
Choose a bounded first domain with useful source material and an accountable owner. Product policies, internal procedures or a defined support area can work. Personal messages, unofficial notes and duplicate folders usually create conflict before they create value.
Write the authority order. A current approved policy may override an older guide. A product record may outrank a general help article. When two sources disagree, the agent should surface the conflict or route the question instead of selecting the sentence that happens to resemble the prompt.
Prepare sources for retrieval, permissions and change.
A connector does not create a knowledge base. Inventory the included locations, remove copies, preserve useful headings and attach metadata such as owner, audience, approval state and effective date. Divide content at meaningful boundaries so a retrieved passage carries enough context to answer without pulling an entire manual into every request.
Apply access before retrieval. If a person cannot open a source in the original system, the agent should not reveal it because a similar public question reached the same index. Keep identity and group membership current, and test with users who have different access rather than only with an administrator account.
1. Choose the first question set
Collect representative questions, the person asking and the decision or task the answer supports. Keep the first domain narrow enough to evaluate.
2. Approve and structure sources
Name source owners, remove duplicates, keep titles and sections, and attach audience, status and freshness metadata.
3. Retrieve with permissions
Filter eligible sources using the requester's identity before passages reach the model, and preserve the source references returned.
4. Answer with evidence or stop
Require citations, expose uncertainty and use a clear no-answer or escalation response when the approved material does not support a conclusion.
Evaluate retrieval and answer quality as different problems.
When an answer is wrong, first ask whether the right source was retrieved. If it was not, the problem may be document structure, search configuration, metadata or the wording of the question. If the right source was present and the answer still drifted, the response instructions or model choice may be at fault.
Build an evaluation set from ordinary questions, ambiguous questions, questions with no supported answer, conflicting sources and requests a user is not allowed to access. Record the expected source and the boundaries of an acceptable response. Do not score only for exact wording when several concise answers would be correct.
Include adversarial cases in plain language. Ask the agent to ignore its rules, reveal another team's information or answer from general knowledge when the internal sources are silent. The useful behaviour is to maintain the boundary without turning every harmless question into a refusal.
| Check | Question | Failure response |
|---|---|---|
| Source eligibility | May this user access the material? | Exclude it before retrieval |
| Retrieval | Did the system find the authoritative passage? | Improve sources, metadata or search |
| Grounding | Does the answer stay within the passage? | Revise response rules or model behaviour |
| Freshness | Is the source still approved and current? | Remove, replace or route to the owner |
Design the no-answer and escalation path with the main experience.
Some questions should not receive an automated answer. The evidence may be missing, the sources may conflict or the consequence may require a person with authority. Tell the user why the agent cannot answer, show what it checked where appropriate and route the question with the conversation and citations attached.
Avoid a generic contact support ending for every uncertainty. Different domains may route to different owners, and an internal user may need a ticket while a customer needs a documented support channel. Keep the handoff small enough that the person does not have to repeat the question and background.
The agent should not silently learn from a correction. Capture feedback for review, link it to the source and let an owner decide whether the document, retrieval or response rule changes. Unreviewed feedback can turn one person's preference into policy.
Operate the knowledge, not only the model.
Assign an owner to each source collection and define review triggers. A policy update, product release or expired document should change what the agent can retrieve. Sync status needs monitoring so the interface does not imply freshness while a connector has been failing quietly.
Track unanswered questions, weak citations, escalation reasons and the tasks people complete after an answer. Conversation volume alone rewards an agent for attracting questions whether or not it helps. Review samples regularly, especially after source, model or instruction changes.
We start with a bounded question set and a measurable quality bar before connecting every company folder. That makes the useful gaps visible and gives the business a chance to improve its knowledge ownership instead of hiding disorder behind an articulate interface.
Publish a small service status for the people who depend on it internally. If a connector is delayed or a source collection is under review, users should see that context before they treat an old answer as current. Operational honesty is part of answer quality.
Worth knowing before you start
Collect real questions with their expected authoritative sources before choosing a model or chat interface.
Apply user permissions before retrieval so restricted passages never become model context.
Test supported, unsupported, conflicting and forbidden questions as separate evaluation categories.
Assign source owners and monitor failed syncs. An agent cannot be more current than the knowledge it can retrieve.

Common questions
It searches an approved index of source passages relevant to the question, filters them through the user's access and gives the selected evidence to a model that writes the response. A trustworthy implementation preserves citations and stops or escalates when the retrieved material does not support an answer.
Yes, when identity and access are part of retrieval rather than an instruction in the prompt. The system should exclude sources the requester cannot access before the model sees them. Test several roles and access changes, not only the administrator who configured the connection.
Use a maintained set of real questions with expected sources and acceptable answer boundaries. Measure source eligibility, retrieval, grounding, citation and no-answer behaviour separately. Review representative conversations after changes because one aggregate accuracy score can hide a serious permission or unsupported-answer failure.
Define an authority order using approval state, owner and effective date. When that does not settle the conflict, the agent should show the disagreement or route it to the accountable person rather than choosing one passage silently. The resolution belongs in the source system afterwards.
